Palm Beach's current Property Appraiser affidavit is revised October 2026. It is titled Confidential Record Request Affidavit for Primary Residence Only. The request asks the office to remove the protected name from appraiser public records. The affidavit includes a notary acknowledgment with physical-presence and online-notarization choices. Official source.

The affidavit asks for owner names and their relationships to the applicant. It distinguishes current employees, former employees, spouses, and children. It also provides space for property control numbers where the home address appears. An additional line covers a tangible personal property account associated with the home address. Official source.

Palm Beach uses a numbered reason-code system on the following pages. The current PDF says every applicant must verify the selected reason code. Some entries call for a badge number, jurisdiction, or license documentation. These are the county form's document requirements. The current statute remains the source for the category wording. Official source.

The first-page header lists the Governmental Center's fifth floor. The return instruction instead directs the affidavit to the first floor, Attention Confidential Records. Both use 301 North Olive Avenue in West Palm Beach. The form gives 561-355-2866 for questions. The return instruction should control the delivery destination rather than the header alone. Official source.

The affidavit expressly requires another request after relocating or purchasing other properties. It therefore should be retained with the household's moving records. The official FAQ separately confirms that the public website offers searches by name or address. No name, property, or address was entered during this research. Official source. Official source.

The Clerk's current indexed redaction request is printed February 12, 2026. Its first page separates the protected employee from a spouse, child, or other protected individual. The form lists the statutory bases beside its checkboxes. It warns that grantor, grantee, and party names cannot be removed from Official Records unless they contain the street address. Official source.

The same Clerk form includes a Congressional Member or Public Officer checkbox. It requires that category's separate statement and notes that city and ZIP code are not exempt. That instruction differs from a promise to hide the entire address. The Clerk's categories, requested information, and statutory conditions must be read together rather than treating every checkbox as identical coverage. Official source.
